— Who this page is for
Who under-eye filler may (and may not) suit
The tear-trough area is delicate, and filler is not right for every under-eye concern. A doctor-led assessment is essential to decide whether it helps you.
Hollows casting a shadow
A genuine hollow under the eye casts a shadow that makes you look tired, and you want a doctor to assess whether careful filler would soften it.
Tired-looking lower eyes
You look tired even when rested and want an honest opinion on whether the cause is a hollow (where filler may help) or something else.
Not sure if it's bags or pigment
Under-eye puffiness or darkness can come from bags, fluid or pigment — for which filler is often the wrong tool. A doctor can tell the difference.
Want a cautious second opinion
You've been offered under-eye filler elsewhere and want a conservative doctor to confirm it's appropriate before proceeding.
— What this involves
What under-eye (tear trough) filler involves
Tear-trough filler uses hyaluronic acid (HA) placed carefully to soften a genuine hollow beneath the eye, reducing the shadow that can make you look tired. Because HA is gradually broken down by the body, the effect is temporary and varies by individual.
This is one of the more technique-sensitive filler areas, so honest assessment matters more here than almost anywhere. Filler helps when the problem is a true hollow. It is often the wrong tool when the issue is eye bags (herniated fat), fluid retention, or pigment (dark circles from skin tone) — in those cases filler can look worse, not better. Your doctor's first job is to work out which problem you actually have, and to say plainly if filler isn't the answer.
It's also worth knowing how this compares with Botox: anti-wrinkle injections are sometimes discussed for the lower-eye area, but they treat movement, not hollows — a different concern from tear-trough volume. Our under-eye comparison guide explains when each is (and isn't) appropriate.
Where filler is suitable, treatment is conservative and reviewed: small amounts, placed carefully, with the option to add later rather than over-fill. The tear trough carries specific risks because of nearby vessels, which is exactly why it should be performed by a trained doctor who explains those risks openly. Our Iskandar Puteri clinic is around 20–30 minutes from the Tuas Second Link, convenient for patients across Johor Bahru and from Singapore. You receive a personalised quote after assessment.

Common questions
Only sometimes. If the darkness comes from a hollow casting a shadow, carefully placed filler can help. If it comes from pigment (skin tone), eye bags or fluid, filler is usually the wrong tool and can look worse. The first step is a doctor working out which cause you actually have.
It depends on the product and how your body breaks it down — commonly several months up to around a year. Your doctor will give you a realistic range, since longevity varies by individual.
They treat different things. Filler softens a hollow (volume); anti-wrinkle injections relax movement and are used cautiously, if at all, in the lower-eye area. Often neither is the right answer, and a consultation is the reliable way to know — see our under-eye comparison guide.
Hyaluronic acid filler can usually be dissolved with hyaluronidase if needed — a reassuring feature in a delicate area. Your doctor will discuss this as part of planning.
