Under-Eye Filler in Singapore

Doctor-performed tear-trough filler for under-eye hollows that cast a shadow and make you look tired. It is not the right answer for every dark circle or eye bag, so every plan starts with a doctor's assessment at Tanjong Pagar, East Coast or Novena. Results vary by individual.

  • Doctor-performed injections
  • Hollow, bag or pigment — assessed first
  • Conservative, reviewed amounts

— Who this page is for

Who under-eye filler may (and may not) suit

The tear-trough area is delicate, and filler is not right for every under-eye concern. A doctor-led assessment is essential to decide whether it helps you.

  • Hollows casting a shadow

A groove runs from the inner corner of the eye down the cheek and casts a shadow that makes you look tired, even when rested.

  • Tired-looking lower eyes

You want an honest opinion on whether the cause is a hollow, where filler may help, or something else.

  • Not sure if it's bags or pigment

Puffiness or darkness can come from eye bags, fluid or pigment — for which filler is often the wrong tool. A doctor can tell the difference.

  • Want a cautious second opinion

You have been offered under-eye filler elsewhere and want a conservative doctor to confirm it is appropriate before going ahead.

— What this involves

How under-eye filler works

Under-eye filler — also called tear-trough filler — uses a soft hyaluronic acid (HA) gel placed in small amounts beneath the eye to soften a hollow. With less of a groove, less shadow forms, and the lower eye looks less tired. HA is a sugar molecule found naturally in the skin; the body breaks injected HA down gradually, so the effect is temporary.

Filler helps when the problem is a true hollow. It is often the wrong tool when the issue is eye bags (fat that has pushed forward), fluid retention or pigment (dark circles from skin colour) — in those cases filler can make the area look puffier or bluer, not better. Your doctor's first job is to work out which problem you actually have, and to say plainly if filler is not the answer.

The tear trough sits close to blood vessels and has thin skin, so it is one of the more technique-sensitive areas for filler. That is why treatment is conservative: small amounts, placed deep and carefully, with a review visit before anything more is added. HA filler can usually be dissolved with an enzyme called hyaluronidase if needed.

In Singapore, dermal fillers are regulated medical devices and should be injected by a registered doctor after assessment. At DrPlus, the same doctor who assesses your under-eye area performs the treatment, at our Tanjong Pagar, East Coast or Novena clinic.

— What to expect on the day

Your under-eye filler appointment, step by step

A first visit combines the assessment and, if filler suits you, the treatment. You can also take the plan away and decide later.

  1. Step 1

    Medical history

Your doctor asks about your health, medications and supplements that affect bruising, allergies, pregnancy or breastfeeding, previous filler and any eye or eyelid surgery.

    Duration:
    About 10 minutes
  2. Step 2

    Under-eye assessment

The area is examined in overhead and side light, at rest and while smiling, to tell a hollow from an eye bag, fluid or pigment — and to check skin thickness.

    Duration:
    About 10 minutes
  3. Step 3

    Plan, consent and preparation

If filler suits you, the doctor explains the amount, technique, expected swelling and risks. The skin is cleansed thoroughly; a numbing cream may be applied.

  4. Step 4

    The injections

Small amounts of HA filler are placed deep, close to the bone, using a fine needle or a blunt cannula. Most fillers also contain a local anaesthetic.

    Duration:
    Usually 15–20 minutes
    Sensation:
    Pressure and a brief sting
  5. Step 5

    Check, aftercare and review date

The doctor checks the result sitting up, gives you written aftercare and books a review at around two weeks, when swelling has settled.

— What to expect

Downtime and aftercare

Most people return to work the next day, though bruising or swelling can be visible for a few days. Simple precautions help the filler settle.

  1. Straight after

Mild swelling, redness and tenderness are common. A cold pack, used gently, can help. Avoid makeup on the injection points for the rest of the day.

  2. First 48 hours

Avoid strenuous exercise, alcohol, saunas and pressing or massaging the area. Sleeping with your head slightly raised can reduce swelling.

  3. First two weeks

Swelling and any bruising settle. Avoid facials, laser treatments and dental work where possible, and do not judge the result too early.

  4. Two-week review

Your doctor checks symmetry and smoothness. A small top-up is only considered if it is needed and safe.

How the result develops

Days 1–3

Swelling is usually at its most visible. Small bruises can appear at injection points.

Weeks 1–2

Swelling settles and the filler integrates. The shadow under the eye looks softer.

Months 6–12

The filler is gradually broken down. Many people need less product at later sessions.

How long filler lasts varies with the product, the amount used and your metabolism. Contact the clinic immediately if you notice increasing pain, skin that turns pale or dusky, or any change in vision.

— Hollow, bag or pigment?

Is under-eye filler right for your dark circles or eye bags?

'Dark circles' and 'eye bags' describe several different problems. Filler only treats one of them well. This table shows how a doctor tells them apart and what usually helps instead.

  • Many people have a mix of causes, so a plan may combine treatments or stage them.
  • If the main problem is not a hollow, your doctor will say so rather than offer filler.
Common causes of under-eye concerns and whether filler is the right tool
CauseWhat you seeIs filler the right tool?
Tear-trough hollowA groove from the inner corner of the eye that casts a shadow in overhead lightOften yes — this is what under-eye filler is designed for
Eye bags (fat pad)A bulge under the eye that is more visible when you look up or smileUsually not; filler can make bags look larger. Surgery or other options are discussed instead
Fluid or puffinessSwelling that is worse in the morning or after salty food or poor sleepNo — filler can worsen fluid retention
PigmentBrown or bluish-brown colour that does not change with light angleNo — pigment and skin treatments are more appropriate
Thin, crepey skinFine creases and visible veins through thin skinSometimes skin boosters rather than filler

— Risks & side effects

Is under-eye filler safe? Risks and side effects

Under-eye filler is generally well tolerated when a trained doctor assesses you first, uses a registered product and injects conservatively. Because the area has thin skin and nearby blood vessels, it carries specific risks your doctor will explain before you decide.

  • Common and short-lived: swelling, bruising, tenderness and small bumps at injection points.
  • Less common: lumps, uneven results, prolonged under-eye swelling, or a bluish tint if filler sits too close to the surface (the Tyndall effect). Most can be improved or dissolved with hyaluronidase.
  • Rare but serious: filler blocking a blood vessel, which can damage skin and, very rarely, affect vision. Increasing pain, pale or dusky skin, or vision changes need urgent review by the treating doctor.
  • Not suitable during pregnancy or breastfeeding, with active infection or cold sores near the area, with a known allergy to the product or lidocaine, or when the main concern is eye bags or fluid.

— Choosing a doctor

How to choose a doctor for under-eye filler in Singapore

The tear trough is one area where the choice of doctor matters as much as the product. These questions are reasonable to ask at any clinic, including ours.

  • Is a registered doctor assessing you and injecting? Any doctor can be looked up on the Singapore Medical Council's online register.
  • Does the doctor treat the tear trough regularly, and can they explain how they reduce the risk of a vessel blockage?
  • Is hyaluronidase available on site in case filler needs to be dissolved urgently?
  • Will the doctor name the HA filler product, and is it registered with the Health Sciences Authority?
  • Did the doctor check whether your concern is a hollow, a bag, fluid or pigment — and tell you if filler is not the answer?
  • Is there a review visit, and do you receive a written plan with time to decide and no pressure to treat on the day?

— Compared with alternatives

Under-eye filler compared with other options

Several treatments are used around the eyes, and they solve different problems. This is why 'tear trough treatment without filler' is a reasonable question to ask.

How under-eye filler compares with skin boosters, anti-wrinkle injections, lasers and surgery

— FAQ

Common questions

Only sometimes. If the darkness is a shadow cast by a hollow, carefully placed filler can soften it. If it comes from pigment, eye bags or fluid, filler is usually the wrong tool and can look worse. The first step is a doctor working out which cause you actually have.

Commonly from several months to around a year, because the area moves less than other parts of the face. It varies with the product, the amount used and how your body breaks it down. Your doctor will give you a realistic range for the product planned.

Usually not. Eye bags are typically caused by fat pushing forward, and adding filler beneath them can make the area look fuller. In some people, a small amount of filler below a mild bag can smooth the transition into the cheek, but significant bags are better assessed for other options, including a surgical referral.

Most people feel pressure and a brief sting; numbing cream and the anaesthetic mixed into most fillers help. Swelling and small bruises are common for a few days, and most people return to work the next day. Plan treatment at least two weeks before an important event.

Hyaluronic acid filler can usually be dissolved with an enzyme called hyaluronidase, which is useful if a lump, bluish tint or unwanted result develops. Your doctor will discuss this as part of planning.

It depends on the cause. Thin, crepey skin may respond to skin boosters or resurfacing, pigment to pigment treatments, and eye bags to surgical options. A true hollow is the one problem those options do not fill, which is where filler is considered.

The aim is a softer shadow and a less tired look, not a change to the shape of your eyes. The result is judged after about two weeks, once swelling settles. Some people need a second small session; others are not suitable at all. Results vary by individual.

They treat different things. Filler softens a hollow; anti-wrinkle injections relax muscle movement and are rarely used under the eye itself. Lines at the outer corners (crow's feet) are a different concern from a tear-trough hollow, and a consultation is the reliable way to tell which you have.
