— Who this page is for

Who cheek filler may (and may not) suit

Cheek filler restores support where volume has been lost. It is not a way to change your face shape dramatically, and it is not right for everyone.

  • Cheeks that look flatter than before

Your cheeks have lost fullness with age, weight loss or exercise, and your face looks more tired or drawn.

  • Deeper smile lines or under-eye shadows

Loss of cheek support can deepen the folds beside the nose and the shadow under the eyes. Supporting the cheek can soften both.

  • Wanting balance, not size

You want your cheeks to look like your own, a little fuller, rather than higher or wider than they ever were.

  • Unsure if filler is the right tool

Sagging skin, puffiness or a wide jaw are different problems. A doctor can tell you whether filler helps or whether another option suits better.

— What this involves

How cheek filler works

Cheek filler uses a hyaluronic acid (HA) gel placed over the cheekbone and in the mid-face to replace volume that has been lost. HA is a sugar molecule found naturally in the skin; injected HA is broken down gradually, so the effect is temporary.

With age, the fat pads of the mid-face thin and shift downward and the bone underneath slowly changes. The cheek looks flatter, and the folds and shadows below it deepen. Restoring support at the cheek often softens those lower areas indirectly, which is why doctors usually look at the cheek before filling a smile line directly.

Firmer gels are usually chosen for the cheek because they need to support tissue, and they are placed deeper than in the lips. The amount is planned against your face as a whole; overfilling the cheek is what produces an unnatural, puffy look.

In Singapore, dermal fillers are regulated medical devices and should be injected by a registered doctor after assessment. At DrPlus, the doctor who assesses your face performs the treatment, at our Tanjong Pagar, East Coast or Novena clinic.

— What to expect on the day

Your cheek filler appointment, step by step

A first visit combines the assessment and, if filler suits you, the treatment. You can also take the plan away and decide later.

  1. Step 1

    Medical history

Your doctor asks about your health, medications and supplements that affect bruising, allergies, pregnancy or breastfeeding, and any previous filler or facial procedures.

    Duration:
    About 10 minutes
  2. Step 2

    Face assessment

The doctor looks at your face from the front and side, at rest and while smiling, to see where volume has been lost and how the cheek relates to the smile lines, under-eyes and jawline.

    Duration:
    About 10 minutes
  3. Step 3

    Plan, consent and preparation

If filler suits you, the doctor explains the amount, the injection points, expected swelling and risks. Your skin is cleansed and a numbing cream may be applied.

  4. Step 4

    The injections

HA filler is placed deep over the cheekbone and mid-face using a fine needle or a blunt cannula. Most fillers also contain a local anaesthetic.

    Duration:
    Usually 15–30 minutes
    Sensation:
    Pressure and a brief sting
  5. Step 5

    Check, aftercare and review date

The doctor checks symmetry with you sitting up, gives you written aftercare and books a review at around two weeks.

— What to expect

Downtime and aftercare

Most people return to work the same or next day. The cheeks may feel firm or tender for a few days while the filler settles.

  1. Straight after

Mild swelling, redness and tenderness are common. Avoid makeup on the injection points for the rest of the day.

  2. First 48 hours

Avoid strenuous exercise, alcohol, saunas and pressing or massaging the cheeks unless your doctor asks you to.

  3. First two weeks

Swelling settles. Avoid facial massage, facials and laser or heat treatments over the area, and try not to sleep face-down.

  4. Two-week review

Your doctor checks balance between the two sides. A small top-up is only considered if it is needed and safe.

How the result develops

Days 1–3

Swelling is most noticeable and the cheeks can feel firm. Small bruises may appear.

Weeks 1–2

Swelling settles and the filler softens into the tissue. The result is judged at the review.

Months later

The filler is gradually broken down. Firmer cheek fillers often last longer than lip filler, varying by product and person.

How long filler lasts varies with the product, the amount used and your metabolism. Contact the clinic immediately if you notice increasing pain, skin that turns pale or dusky, or any change in vision.

— Is filler the right tool?

Cheek concerns and what usually helps

Several different changes can make the mid-face look older or more tired. Filler treats volume loss; it does not tighten loose skin or slim a wide face.

  • Many people have more than one of these, so a plan may combine treatments or stage them.
  • If filler is not the right answer, your doctor will say so.
Common mid-face concerns and whether cheek filler is the right tool
ConcernWhat you seeIs cheek filler the right tool?
Volume lossFlatter cheeks, a hollow below the cheekboneOften yes; this is what cheek filler is designed for
Deep smile lines from lost supportFolds from the nose to the mouth that deepen as the cheek flattensOften, by supporting the cheek first
Loose or sagging skinSkin that folds or drapes rather than looking hollowPartly at most; skin-tightening options or surgery may suit better
Wide lower faceA square jaw from a large chewing muscleNo; jaw slimming injections are a different treatment
PuffinessSwelling that changes through the dayNo; filler can add to puffiness

— Risks & side effects

Is cheek filler safe? Risks and side effects

Cheek filler is generally well tolerated when a trained doctor assesses you first, uses a registered product and injects conservatively. Your doctor explains the risks before you decide.

  • Common and short-lived: swelling, bruising, tenderness and a firm feeling at the injection points.
  • Less common: lumps, asymmetry, or filler that looks too full. Most can be adjusted or dissolved with an enzyme called hyaluronidase.
  • Rare: infection, or a delayed swelling reaction weeks or months later.
  • Rare but serious: filler blocking a blood vessel, which can damage skin and, very rarely, affect vision. Increasing pain, pale or dusky skin, or vision changes need urgent review by the treating doctor.
  • Not suitable during pregnancy or breastfeeding, with active skin infection near the area, or with a known allergy to the product or lidocaine.

— Choosing a doctor

How to choose a doctor for cheek filler in Singapore

These questions are reasonable to ask at any clinic, including ours.

  • Is a registered doctor assessing you and injecting? Any doctor can be looked up on the Singapore Medical Council's online register.
  • Did the doctor look at your whole face, not only the cheek you pointed to?
  • Will the doctor name the HA filler product, and is it registered with the Health Sciences Authority?
  • Is hyaluronidase available on site in case filler needs to be dissolved urgently?
  • Are the risks explained, including the rare risk of a blocked blood vessel and what the clinic does about it?
  • Is there a review visit, and do you receive a written plan with time to decide?

— Compared with alternatives

Cheek filler compared with other options

Several treatments are used for the mid-face, and they solve different problems. This table describes them generally; your doctor explains which suit you and which DrPlus offers.

How cheek filler compares with skin tightening, collagen stimulators, skin boosters and surgery
OptionDesigned forPoints to know
Cheek filler (HA)Lost volume over the cheekbone and mid-faceVisible after swelling settles; temporary; can usually be dissolved
HIFU or radiofrequencyMild skin laxityTightens rather than adds volume; results build over months
Collagen stimulatorsGradual, diffuse volume lossResults develop over several sessions; not dissolvable like HA
Skin boostersSkin quality, hydration and fine textureDo not lift or add shape
Facelift surgerySignificant sagging and excess skinPerformed by a surgeon; longer recovery; referral if appropriate

— FAQ

Common questions

Commonly around a year, and sometimes longer, because the cheek moves less than the lips and firmer gels are used. It varies with the product, the amount used and how your body breaks it down. Your doctor gives you a realistic range for the product planned.

It should, when the amount is planned against your whole face. Unnatural results usually come from too much filler or filler placed only to make the cheek higher. A conservative first session with a review is the usual approach.

It depends on how much volume has been lost, your face shape and the product. Filler is measured in millilitres, and many people start with a small amount per side and review before adding more. Your doctor recommends an amount after examining you.

Often, indirectly. When the cheek loses support, the fold from the nose to the mouth deepens. Restoring cheek volume can soften that fold, and the doctor may treat the cheek first rather than filling the fold directly.

Most people feel pressure and a brief sting; numbing cream and the anaesthetic in most fillers help. Swelling and small bruises are common for a few days, and most people return to work the same or next day.

Hyaluronic acid filler can usually be dissolved with an enzyme called hyaluronidase if there is a lump, asymmetry or a result you do not like. Your doctor discusses this as part of planning.

We do not publish patient result photos. Singapore's rules on medical advertising do not allow them to be used to promote treatments, and results vary between people. Your doctor explains what change is realistic for your face at the consultation.

They treat different things. Filler replaces lost volume; HIFU tightens mildly loose skin. Many tired-looking faces have some of both, which is why the doctor assesses the cause before recommending either, or a staged plan.
